Replacement Documents (Lost Boat Title)

Application for Boat Title/Registration (TC96-184) (See Example)

Complete the following:

  • Boat Information Section
  • Owner/Buyer Section, including current owner’s name, SSN and address
  • Check the appropriate boxes for the lien information
  • Signature and notary

Fees:

  • $6.00 – Application notarized out of our office
  • $9.00 – Application notarized by the County Clerk’s office

Lost Title

What you will need:

Application for Title/Registration (TC96-182) (See Example)

  • Type of application desired
  • Vehicle Identification Section
  • Odometer Disclosure
  • Owner/Buyer Section, including current owner’s name, SSN, and address* (this address is where the new title will be mailed)
  • Check the appropriate boxes for the lien information
  • Signature and Notary

Fees:

  • $6 – Application notarized out of our office
  • $9 – Application notarized in County Clerk’s office
    (Owner must be present with Government issued photo ID)

Lost Registration

What you will need:

  1. Affidavit for Replacement (TC96-167) must be signed by the registered owner of the vehicle. If the vehicle is owned by a company, an officer or agent of the company must sign for the replacement.
  2. Proof of current Commonwealth of Kentucky liability insurance
  3. The owner’s signature must be notarized. Picture identification is required to have documents notarized in our office.

The registration must be current. We cannot replace expired registrations.

Fees:

  • $3 – Affidavit notarized out of our office
  • $6 – Affidavit notarized by the County Clerk’s office

Lost Plate or Decal

What you will need:

  1. Original Certificate of Registration. (Photocopies are not acceptable.)
  2. Proof of current Commonwealth of Kentucky liability insurance
  3. Affidavit for Replacement (TC96-167) must be signed by the registered owner of the vehicle. If the vehicle is owned by a company, an officer or agent of the company must sign for the replacement.
  4. The owner’s signature must be notarized. Picture identification is required to have documents notarized in our office.

Fees:

  • $6 Affidavit notarized out of our office
  • $9 Affidavit notarized by the office
